Born as a response to the findings of Improving Maternal Health in Harris County: A Community Plan, 2018 released by a steering committee of health leaders convened by Houston Endowment, the Healthy Women Houston (HWH) effort was created to provide a comprehensive and integrated system of maternal care by engaging women in their community and providing them with the medical care and social supports they need to have a healthy pregnancy.

Healthy Women Houston is a vertical integration collaboration of multiple community partners, including Avenue 360 Health & Wellness, Community Health Choice, The Council on Recovery, Harris County Domestic Violence Coordinating Council, HOPE Clinic, Interfaith Ministries for Greater Houston, Memorial Hermann Health System and more.

The collaborative was formed to address the rising rate of maternal mortality and morbidity in the Houston area. HWH brings together key healthcare and social support providers to create a new system of care for women. HWH integrates health, behavioral, and social supports for pregnant and postpartum women, and especially women at higher risk of maternal morbidity and mortality.
